Nobody knows. The ilvl drop isn't an issue unless you fall below i170 to enter Alex NM, in which case just switch to the appropriate accessories and then switch back to i150 when you get in.

then what will be the difference on running with ilvl 170-190 acc? besides more dps?

That's all. The reason you wear i150 in the first place is for the extra DPS, if all you want is more health then go pure VIT accessories.
