This has never been an issue in Mabinogi and it's been available since like 2003. Also it doesn't have blood. Neither does FFXIV.

Here's a mind breaker for you. You can get gay-married in Mabinogi, as two 10 year old girls or boys. You can also have a family consisting of nothing but 10 year olds.

There is nothing illegal about it. The only reason games shy away from this is because of overbearing "childporn" laws and Roleplayers who take advantage of the fact that the video game is "real enough" for them. I'll post this again:

Accusations Regarding Child Pornography in Second Life
by Honored Resident Robin.Linden on ‎05-10-2007 05:32 AM
What’s going on?
On Thursday May 3, we were contacted by German television network, ARD, which had captured images of two avatars, one that resembled an adult male and another that resembled a child, engaged in depicted sexual conduct. Our investigations revealed the users behind these avatars to be a 54-year-old man and a 27-year-old woman. Both were immediately banned from Second Life.

During a subsequent interview with ARD’s Report Mainz documentary on Friday May 4, which aired Monday, the reporter presented photographs that he said were found in Second Life and that appeared to include sexual photographs involving a child.

...
The point is, there will always be bad actors, and keeping your game away from them will never be possible. Look at how little of a dent SE makes into getting rid of RMT.

If you give players the option to play as children, the first thing you do is make sure the body is fused with the gear so modders can't "declothe" them. The second thing you do is remove "offensive" emotes that could be misconstrued as sexual acts.

In Mabinogi there was a "pelvic thrust" part of a certain animation, and because you could interrupt all the emotes with another emote by rapidly firing them off, it gave the impression of grinding into other players. There was also a way to interrupt the "resting" animations to make it look like your character was tea-bagging the another player that is sitting down. These kinds of people will always exist.

And quite honestly, being an asshat online has far more reaching negative consequences for your existing and potential future careers, that people who go around spreading "mmololicon" images are likely to get their ass kicked for realsies by anyone who makes that connection in the media to the real person.
