The lag I've been experiencing has been making this game damn near unplayable for me. I've done everything I can to get it to go away, but at this point its the game. No other game I play has this issue. This needs to be addressed.

I have been experiencing the same thing, mostly in dungeons, which isn't so great as a healer. :/
I'm really sorry to say this but Get used to it folks. This has been ongoing and will likely continue. It affects every service at some point and time. When I had Comcast I had this issue, and then when I changed to Verizon to try and alleviate it I unexpectedly began to rubberband/lag spike AGAIN. If you want to enjoy what you pay for then you need a VPN especially if you can't get anything other than the two net services I listed. And yes ps4 and PS3 can VPN I have and still do it currently until we get lucky again and can use our own connection by itself.
I really wish I had a better solution, I wish that I could offer more than a bandaid. But the powers that be have a chokehold on our connections, and they don't care at all about us trying to enjoy a game.

I can't post detailed reply ATM due to being at work and writing from my phone. But it requires you to have access to a computer/laptop (macs included). From your comp you set up the VPN server and connect an ethernet cable from your PS4/PS3 to your PC.
