This happens to me, usually in the CT raids. I explain and stress how to avoid one hit kill mechanics. I end up derping and not avoiding the one hit kill mechanics. -_-
"So when KB rears up and the text shows a roar,run behind a comet immediately! Otherwise, you will die." *Gets so focused on healing something that the mechanic goes unnoticed till the last second....*
"make sure that when the floor turns red and blue that you alternate the colors red -> blue -> red -> blue every time it appears to change the debuffs on you." *Ends up with a second stack or in the rarer cases, third+death for not paying attention*
"Make sure that you turn away for Immortal Gaze - and if you don't and get Doom on you, run to a glowing pad to get it off before you die"
*Doesn't notice the Doom and dies*

My DPS shame.... every time I'm on melee DPS and I forget I'm allowed to LB.
Meanwhile, in party chat...
"lb"

Me: *Crickets on the LB front*
._. *sends self to the shame box*
I wish I could remember my derps, I'm sure there have been so many its not even funny. Problem is I main Scholar, so my DPS derps are few and far between I guess, but healer derps, I got lots, lol. But I do remember this one time, I was farming Labyrinth for my Dragoon boots; so obviously I went as a Dragoon. Well during the behemlth fight I used jump to try and attack him and return behind the meteor, but I timed it wrong and died. That would be a derp all on its own, but it gets better. I had one of those wiping parties and we wiped 2-3 times, and wouldn't you know, I did the same stupid mistake every run, lol, maybe, no?
During the start of Howling Atomos phase of World of Darkness, as the party started the attack, happened to run back over the jump pad, sending me back to the beginning of the room just in time to get locked out and miss the whole fight.
Stop it, why are we so similar?! xD
I totally forgot about that, but I can't think of a time when I actually used the melee LB on time xD
I guess it's just a typical ranged player thing, since neither BRDs nor Casters are usually allowed to touch it, and healers use it in very different situations^^°
Hm..something else I just remembered:
I tend to brain afk when someone else messes up :x
Most prominently back when I was still trying to get a T9 clear (no static so always with randoms); I was super efficient at placing meteors, but because players always changed, many others weren't.
So sometimes a stray meteor would end up somewhat close to the middle where we dropped the golems. Not a problem actually, I just went a few steps away to have enough distance from the meteor.
Then I'd stand there staring at the stray meteor, wondering if the distance was enough and why the other player didn't like nice, perfect patterns.
...until I got crushed by a freefalling golem.
Not once, not twice but that actually happened 3 times in a row xD
I was really ashamed at the time^^°
To contribute actual mess ups instead of just commenting on others', I recently did Titan as DRG for my relic. Made almost every mistake I could, but at least I didn't get hit by landslide. Running from on bomb explosion to get hit by another, thus taking 2x damage? Check! Using Jump as Weight of the Land goes out? Check! Was a pretty bad performance on my part. . .
So I admitted to a friend that I'm pretty bad about using DPS buffs since I always want to save them "just in case" (healer mindset).. Was properly shamed, so I was determined to do better. Well, later that day, we headed to Brayflox NM so I could level my dragoon..
I tried to use internal release whenever it was up, keep dots up, and not pull hate.. felt like I didn't do too terrible a job. Run was smooth and all, but I ended up with 3 pieces of gear at 99% spirit bond. We headed to Costa del Sol to finish that off.. then right after the FATE..
"You do know you can use jump, right?"
That realization that I didn't use it a single time through the whole dungeon (or earlier FATE farming I did). ;_;
