Nitoryuu is dangerous and impractical, and the only time they ever REALLY did it was with a katana and Wakizashi, and that's if they needed the latter for parrying.
Or disemboweling themselves. That was a thing too.

It shouldn't, but samurai's have used two swords before. It's not entirely impossible and i for one can see it happening.
