This is why its good to always have the same keys in the same spots for every class, as much as you can, so muscle memory doesn't screw you.

I just forget every time that you'll stand still for a while, when casting LB. Bad if you're on Titan EX (no melee [alive]) wanting to destroy the superbombs // damge Titan with your LB, eating all those plumes.
My derps:
Dotting an add, but using Bane/Fester on another one without any effect.
Use Spur/Rouse at Titan EX (or similar) while phase transition and your egi not been able to attack something. Or use it to do a 180° degree while dodging landslide.
Use Contagion or put new DoTs on that 5%-health-add.
Raging Strikes + DoTs + Contagion just before Ifrit Ex's last nails appear. Hurry up, folks.
Swiftcasting Bio/Miasma2/Ruin/Ruinra.
Having my Egi attack Bosses while they are invulnerable (while add phase for example).
To my defense it was a 2am dungeon run with my pugilist. Our healer was rather sadistic in a way that he´d afk half the time and laugh watching us die twice ( Lalas are evil). We had a rather derpie dragoon in the making and my friend on his warrior (who hasnt been playing tank in a long long while)... at some point he throws in "hey i could use some help" - probably adressing the healer - i tried to sound smart - "hey let me throw you a goat to help you out." He ended up laughing so hard he messed up and died. We took it with good humour saying maybe next time i should throw a sheep or bunny.
Also on my ninja forgetting that perfect dodge - only dodges physical attacks.
I would have died if you threw me a goat too :v
I would sooooo hate you for this one XD
So my friend one day needed to kill the FATE at Boulder Downs in Coerthas. You know, that one big guy at the rocky zone at the south-west side of Coerthas' map.
Aaaand so I hurried to help her out. So I warp on the Camp and start running, and then wanna go faster so when I get close enough to the FATE down its north side, I just have to jump down the cliff to get there immediately, I would end up with 1 HP from fall damage of course, but my friend was a Healer so meh! I JUST JUMP!! And then a ninja Drake thing catches a glimpse of me while Im midair and aggroes me...
I was a DRG that just literally died from taking fall damage on a jump... OTL
My friend laughed hard tho... also the FATE completed right before I touched ground so she saw my derp in all its gloriousness.
Gone to use apoca on a tank before Akh Morn, lost track of what hotbar i was on and accidentally hit Aetherial Manipulation. Much dead BLM followedActually recorded it for everlasting shame

That is SOOOOO awesome! I wish I had accidentally done that at one point. I may still work it in to mess with my static.
Repelling shot into Titan's plume. Granted, it was because someone screwed up the stack, but eh. Could've dodged normally, after all. Also, on that note, the screwed up input didn't go through and had me die to the plumes. Ouch.
And of course, untimely accepting a raise just as Titan brings down the platform. And me with it. Yeah.
I have been working on T9 I almost have it done ( got to less than 5% of Naels health left ) it was getting late and after a few hours or so I had one of those big facepalm moments. Stardust comes out the meteor is placed than Iron chariot is cast I get the marker my mind is like oh I have a meteor to place so I went to the edge and proceeded to treat it like a meteor placement.... ended up solo taking a thermionic beam to the face followed by a short nap after which there was much laughter at my expense ( I was laughing too ).

As someone who joined for dual-wieldy NIN goodness but has since picked up DRG as a secondary main, I'm surprised it took me until this week to have my first LOLDRG moment...albeit a weird one that I'm still trying to figure out the exact details of.
I was in WoD helping some guildies with the CT weekly, running DRG to try and get the chestpiece (last piece I needed). We got to the first boss, and after breaking the hourglasses the first time, I positioned myself at what I'd HOPED was a safe place to use Spineshatter Dive from to get back on the boss. I misjudged and ended up locked in place at the very front tip of the pointer finger's field effect, even started laughing at myself in chat...and then noticed that I wasn't dead. I'd somehow landed just far enough into the field effect to get frozen, but NOT far enough in to get hit with the insta-kill. Still not sure how, and I don't plan to test my luck like that again if I can help it.
Don't get me wrong, I still call this a fail. Just also a fail that the game happened to take pity on and not really punish me for...for some reason.
