Bahamut lived and died a LONG time ago and was summoned back as a primal by his descendents, the Meracydians, then trapped in a moon for a few thousand years. He was a living dragon that was summoned back in primal form just as Good King Moggle Mog was a living king that was summoned back as a primal. All you need to summon a primal is a prayer or desire, a target to pray too, and enough aether to give your prayer physical form. An example would be if one day we get access to aether on Earth, then enough prayer to George Washington or Abraham Lincoln with enough aether would summon them back into the world in primal form.

I'm pretty sure that Bahamut is one of Midgardsormr's seven children, but not one of the two mentioned still in Eorzea since he's technically been dead for several millennia and imprisoned as a primal for several millennia more. We're aware of Nidhogg so far, so the second should probably be coming into our storyline during this fall before Heavensward and may be either Fafnir or Tiamat.

The Ishgardian dragoons are dragon slayers, so I'm pretty sure that's the first mistake. My money's on that dragons and people at one point were allies until someone broke the truce escalating centuries or millennia long war between mankind and dragonkin. If that's the case I think the start of the war was when Shiva fell in love with that Dragon who may or may not be Nidhogg.
