Wouldn't it just be best to judge the situation on whether to save swift cast or not? If you are running with a team you are familiar with you should be able to communicate well enough to know when you will need to save it and when you wont. If it's with people you don't know you could just feel it out and monitor how fast healers are doing their job, if skeptical just save it for a res.
There's no way to "judge" when you should save Swift Cast or not. The need to use Swift Cast for resurrect doesn't occur till anyone actually messes up and dies. This will lead to two situations:
1) You used Swift Cast earlier to maximize your DPS
2) Swift Cast has been off CD for a long, long time
Much like your buffing skills (Raging Strike), you try to keep it on CD to maximize your DPS.
Learning very quick that my rez as summoner is never needed in crystal or syrcus tower o.O its to freaking easy. Easy mode like easy peasy.

DPS wise, using it on Shadow Flare is less of a gain compared to having to move and using SC on Bio II or Miasma or SF, so in mobile fights it can be best to save SC for movement. That being said, sometimes you can be sure you won't need it within the next 60s and should use it on Shadow Flare, which is a large portion of the fights.
Priority for SC: Opener > raise (if needed - as stated multiple times SMN is the last resort for raising in a coordinated group) > forced movement > Shadow Flare over raw cast if no forced movement in the next 60s

To play devils advocate, sometimes the healer doesn't res immediately because they need to pay attention to the tank hp, or that they are waiting for a better time to res. I don't exactly trust df pugs to know how to accept raise at the right time in order not to die to the next aoe attack and costing me more mp to raise them again so I'd rather time my raise for them.
