Well, I decided to tinker with my settings first. I lowered a couple things, but I just noticed something new: http://beelibuzz.com/random/Bee-Shadow.jpg What is causing these blocks of shadow on my character's face? In game, it's all over the place and very annoying. Is it the shadow setting? It's pretty distracting, and I'd at least like to alleviate that graphically.

That's the bad implementation of the self-shadowing or shadow maps in general. SE was probably too lazy to implement some complicated technique like PSSM. So the only way to reduce that is increasing the shadow resolution or completely disable shadows (ingame options).

Augh, that's terrible. :/ Oh well, I'll see what I can do.

I should mention though, I just went in game to check out some of the new settings I changed, and the lag/stutter/sluggishness/whathaveyou is much, much worse in full-screen mode for me. All at once different sections of my screen are refreshing the image at different rates, so I've got these patches everywhere that are jittering. I'm going to try it in windowed mode again and see if that makes a difference.

Fullscreen issues are in 99% of the cases caused by the settings in nv control panel/amd control center(not only the settings there but also the profiles). That's because all those settings only get applied to games that run in fullscreen mode :P
That's just my observation though. Might have been coincidence.

If you have narrowed the issue down to HDD stuttering you can mitigate some of that (not all of it) by nuking your preferences. It works for all kinds of HDD related stuttering.

In your Documents / My Games folder there will be a Final Fantasy XIV folder. Inside that is a bunch of stuff. Create a folder on your desktop, and remove everything in the folder into the temp folder on your desktop. Once that is done, move only your user folder back in to the FF folder, and run the game. See if that helps.

Hopefully this screenshot is self explanatory enough for you to understand.

    Try running fullscreen again.

If it's resolved, you can turn your settings back up.

Note: If this solves it, and you experience a new issue (fps randomly tanking, specifically; from 60 FPS to 30 FPS with no variance in between), then let me know; you'll have to force Triple Buffering to resolve that and it can't be done through the Nvidia Control Panel.

Also, as someone said above, measure your FPS with FRAPS: http://fraps.com

I have a 460 also and getting an interesting issue. Whenever I am in a more populated area when I log in, it most of the time it crashes itself, giving me a Windows error. I have completely reloaded the OS, reinstalled the game, reinstalled my drivers, set it at not too high of settings. I cannot figure out why it constantly gives me this.

~3% of the hardware shipped has some issue(not always critical). If a clean install of everything with newest drivers doesn't help, it's best to pinpoint the broken part and get it replaced. But since we are talking about Fatal Fragile XIV it might also be a problem on SE's side, caused by a broken entry in the database(so you better also try it with a new character :P)

This was the same issue when I had a GTX260, and carried over to 460. I reloaded Windows for other reasons, but after reinstalling it along with reinstalling my drivers, it still does it. FFXIV is the only game that does it, no other PC game I play acts to this. I am assuming it is something on SE's end, or possibly something on my end, but I am having a hard time figuring out what.
