It's just a matter of playing together in my opinion. When I play with random players, I check out the tank first. If he is a lot below my Item level, I give him a bit more time before I attack anything, use quelling strikes and even if I am AOEing, I take care that my main Focus is the target of the tank and the rest of the group. 95% of the time, this is enough. The rest is mediocre tanking.
I tend to pop all my buffs then quelling strikes. And that's usually it for QS. But yeah still manage to pull hate at times. But yeah poping QS at start has been ingrained in me by titan's fist. One time I didn't... blam.
I see QS as utility to help the tank get his hate combos out at the start.
I think the better solution is leave the dps ate the mobs they aggroed and continue to hold aggro on your marked 1 mob, switching to 2 after 1 dies. If 2 can't get aggro back because dps does not stops dps, well, leave him have fun with the mob and don't waste your TP for him.
Healer that get aggro, is different, if he gets aggro because its job, help him. If he gets aggro because he will cure2 you when you just had 1 hp off, well, it is same as the dps. If they done that on mistake, next mobs they will be more careful, if instead is a continue pull of mobs, it means they doing that on purpose, or because their armor ilvl is 200 but them understanding of aggro mechanics is ilvl 0. But cases, you safe to ignore them at all and make your job.
This sounds harsh, but it's fair I think. I'm not saying every dps / healer should be punished for not understanding their part in enmity control, but this will simply give them the opportunity to learn by experience. Also, it's not fair for every tank to be expected to be exceptional at keeping enmity just because quite a lot of them do happen to be. EDIT: That said, it's just more fun to play to the very best of your ability. As a healer, if people are requiring more heals than they "should" do, I couldn't bring myself to just let them die when I know I can pull something off :/
