Respectfully disagree about accuracy. I count the amount of times I've seen Energy Drain miss dreadnaught on a single hand in the 15+ T4 runs I've participated in and it's rare to see Miasma miss, and Holy stops things dead in their tracks in P1 and P4.

All WHM I've ran with, and even running on my own WHM, report misses on every attack. On every turn... It may be that 341 accuracy give you like 5% (or something ridiculously low) chance to hit.. 435 accuracy is obviously for 100% hit chance.. But I suppose you could occasionally hit something with the minimum.
