Every instant we have is like this. Sometimes I have to hit rampant 3 to 4 times before it fires. As the tip says instant, I should be able to hit the key and it fires no matter the animation going on.
All that is Fine and dandy, but if I use a skill and its effect does not go off then the skill was never actually used, therefore should not be on cooldown.Hollowed is already overpowered as any thing in this game, and just because you do not understand how the game works you think its broken. It is working as intended, you simply are using it too late. There are very few places where HG is really needed, and the use of HG is planned for them, its not a we messed up i'm dead in .5sec lets save the day skill. If it would fire instantly when you press the skill then the skill would be gamebreaking and nerfed, you cant have both. If enough cry about the delay and they make it fire instantly, it will simply become a weaker skill to balance out the fact that it can be used for more things than it was designed to be able to stop.
I see two issues mentioned in this thread.
One is the wind up coming from animations when you use an instant skill.
The other being the issue that some skills go on cooldown even when they did not take effect such as benediction going on cooldown without actually healing anything.
The first im perfectly fine with, it just makes the game a bit more on your toes. You have to think ahead and make tactical decisions.
The second however is a lag issue and should be dealt with. I see nothing preventing the server from correcting the fact that your skill went on cooldown when it was in fact cast under circumstances it should not have been able to (i.e. when you are dead).
It works fine for me, and others. So saying things like "it's completely broken" are just plain wrong.
Learn to predict when you'll need it? Seriously?
Yeah sure HG is perfectly useable when you are planning to use it ahead of time, you might want to read my post again though. I said its useless as a panic button, you know.. when your hp drops in the triple digits and your not sure if the healer will keep be able to keep you up? Yeah not gonna matter... 3s between cast and animation before effect is way too long, and many tanks will die in the situations where that happens. You very often get into a situation where you use the ability, the animation goes off, you die, and then the CD starts. wtf? This skill is completely broken for what it should be doing.
If I die with HG activated then the CD should not start. Its just another example of the horrible animation delay system they put in this game.
It's fine as a panc button. I use it as a panic button. If you're focused enough you can notice when you'll need it.
I know this because it works for me almost always in these situations. There are times when things get a little too hectic, or you lag at the exact time you needed it, etc... but it works much more often than not for me.

I hate to bring this up but it doesn't look like you have any experience in Coil. Inbound damage predictability in coil is incomparable to the rest of the game.It's fine as a panc button. I use it as a panic button. If you're focused enough you can notice when you'll need it.

Get back to me when you face a 3 or 4 stack Cadaceus that hood swipes and auto attacks you for ~5000 damage and your healers are on the move to avoid a light panel. I want to see you get Hallowed Ground off in time in these kinds of situations where it's most needed. And this is just the turn 1 boss.
