You should get an email from GMG titled "Your FINAL FANTASY XIV: A Realm Reborn (NA) prepurchase". Your email should look like this; http://i.imgur.com/vUekXMo.png. The keys will be under "Activation Keys" at the bottom of the email.

Simply log into the game and when it prompts you to go through the steps of registration just enter the 20-digit code that the email has given you. Follow the next steps and you'll be in the game again.
