He can call them up and explain the mixup and they might help him out. Amazon is usually nice about it. But if that fails he should talk to SE about it, they may give him a refund or activate the CE on his account providing he has proof.
Square just needs to do like Blizzard. Give us the option to upgrade by using the code we DID get. No reason they can't allow us to do just that.
1.0 players had NO WAY to get the Cait Sith Doll and the Mog hat otherwise. This is ridiculous.
It's not that hard to let us upgrade by using the code.

No, but honestly, I figured that using a code to upgrade an account would be a given by now. I think this mess could have been avoided had this been mentioned on the pre-order page or in the email they send when you order the CE.
Fair enough, it was on the forums, but not everyone browses them. Should have been on the product page itself.
I do hope they will either add an option to upgrade via a code, or simply issue refunds, I suppose.

No refund on amazon....explained the situation. Can't because the code was already sent and that means it's been used. Off to square enix now....
