    My SNES cartridge also says "II".

    However, let's take a look at every subsequent release:

    Final Fantasy IV (Final Fantasy Chronicles on PSX)

    Final Fantasy IV: The Complete Collection (PSP)
    Final Fantasy IV (iPhone)
    Final Fantasy IV: The After Years (Wii)

    Looks like there's no question of what should be the correct title number when you refer to this game.
