We've also had a situation where a Prism'd Esuna botched a run when used after a jump, but like it's been pointed out, save your Shroud for nails.
I've seen this before as has anyone who has done Ifrit X enough. I always thought it was a hate bug as well.
As WHM, I had this experience once: do literally nothing from the time he first jumps until he lands and incinerate complete yet still go from green to flashing red AFTER incinerate. Fieval's response above would explain this.
So this is the scenario?
WHM uses Shroud of Saints, reducing their enmity by half
Ifrit jumps, hate reset for all
Ifrit completes dashes and lands, targets someone other than WHM, uses incinerate
Shroud of Saints wears off and WHM receives the enmity they shred back which of course is much higher than PLD's initial voke/flash
This is the only plausible explanation I've come across and may just be the reason for this as I cannot specifically recall this happening to anyone other than a WHM.

Happened to our WHM a lot and we couldn't figure out what she was doing to cause it. Vex has hit it right on the money. Not really a big deal, she would die off in a corner then tank gets hate back and we rez and move on... This shouldn't wipe any decent group
It's working as the ability was intended, but that doesn't mean it shouldn't be adjusted. A simple resetting of the enmity stored in SoS during a hate reset fixes the problem. It's likely just a situation the devs had not considered (stored enmity and hate resets), but there is an easy enough workaround of not using the ability.

I can see that side of it as well. After all it only says,
"Temporarily reduces enmity by half, and gradually restores MP while effect is active."
If you believe it should be a clean reset after, maybe submit it as a bug / suggestion to SE on their website?
