The "GPU VRM Voltage In (VIN/+12V) [V]" values look not good. They go down to 11.562V. Reseating the graphics card may help. Open your PC, remove the power connector from the graphics card and remove the graphics card from the motherboard. Then clean the contacts with some alcohol and put the graphics card back and connect the power connector to the graphics card.
If it does not help then propably the PSU is old/too weak and it cannot provide enough energy.
