You can see the indent next to the mouth casts a shadow on both. Some polygon tweaks sure, but I think it comes down to Miqo having "fuller" cheeks than some others, so their models curve in more toward the bottom. I do think it is even more apparent on mouth 4 though, as the mouth is narrower so widens that "dip", thus giving you a constant puckered look.
It has never been more obvious. There was a slight indentation at the corners of the mouth, very natural, that never created that wild, huge crease.

They completely changed the shape of the mouth and did something to the cheeks next to it, so that instead of small dimples there were huge gaps. I don't think it was intended that way, I'm sure it was someone's mistake.
