-Avatars are the 13th reflection of their Reaper's soul. This might contradict Golbez being our 13th reflection, which is all but confirmed, given how their shadow "comes out" of us during Lunar Subterrane dungeon and their face is never shown and they are suspiciously unvoiced in a voiced cutscene....
Or was there something about Zenos being an outlier, who searched for and then successfully subjugated a particularly powerful soul to serve him, meaning he enslaved another person's reflection to be his reaper avatar?
(Though that could lead to the kind of funny implication that his own reflection wouldn't be powerful enough for his taste?
I don't mean to say he knows the nature of the reaper avatars (quite frankly I don't remember) but it would be funny if he initially channels his own reflection and then - perhaps without knowing it's literally a part of his own soul - he just goes "nah, you're too weak" and throws it away.)
Sorry, I don't really remember what the details of Zero's story as an avatar are.

The extent we got is "Fandaniel summoned her for him", I think?? And besides, Zenos was not a reincarnated Ancient, so he... wouldn't have shards in the other reflections in the first place, would he? Although... what does that say about 'duplicate' NPCs (e.g. Rowena, Gerolt) we run into in the First, then...?

You are correct that he did enslave someone else to become his avatar. It would depend on if you consider the souls that were inside the new life made from the 2nd sacrifice as Ancient or not. As if they were freshly minted souls then they would have been sundered but not have the trauma of the Final Days seared onto their soul. If they were preexisting souls that were in the Sea during the Final Days then I'm not sure if they would have said soul searing. Then you have those that died during the final days that I want to say got sent to the Metiea soul egg pouch and thus wouldn't be part of the equation. We then have to remember that the Star probably made new souls post sundering as a good chunk of souls were in purgatory do to being stuck inside Zodiark. Where any post sundering souls might not have any reflections as each Sea is its own separate thing. The problem is unless the person wakes up to the Echo we won't know what kind of soul they have.
Zenos on his own is an oddball to begin with as he's the great grandchild of an Unsundered.

I don’t believe Ancients will only be reborn as other Ancients. If you have the Echo, I took that as meaning that you are a reincarnation of an Ancient who was alive when the Final Days happened. Your past self could theoretically have seen the original starfall during the Final Days and then died during it, as I’m sure many if not most people did, and then your soul was split while in the lifestream when the Sundering occurred.
People like G’raha still have the expected density of souls (though he has even more because time travel shenanigans) even if they don’t have the Echo.
As far as reapers, that was put forward as a theory in EE3, but it might not necessarily be the case since I was under the impression that our 13th soul shard was the OG Golbez and that soul is locked in memoria.
