The rating-based achievements are awesome—the titles mean a lot more if you have to actually be exceptional to wear them.

This statement tells me that you either already have the achievements and don't care if anyone else gets them, or you're a nodding yes-man who agrees with everything Squeenix does regardless of how inane it might be.
I have Mahjong Enthusiast, but not Mahjong Master (getting there), and I have disagreed with almost every decision Square has made for the last several years, including decisions regarding Doman Mahjong. (Why are Novice room time controls so slow?)
The Mahjong titles are fun oddities—they're some of the rarest titles in the game, rarer than every Ultimate title and several completely luck-based nightmare grinds, because they combine an unpopular side activity with a high skill requirement. When I see someone wearing them, I instantly know that they're one of those weirdos who log into a catgirl simulator to try unnecessarily hard in a board game for old people, and I smile; I'm one of those weirdos, too!
The rarity of the titles lets me make a statement about myself, and I think that's neat; if they could be acquired by mindless grinding, they would mean less to me, and probably to a lot of other people wearing them. I want the opportunity to work hard for something.
(Nothing is stopping anyone from simply learning the game and playing better, anyway.)
