I'm having the same issue. It seems like it just refuses to properly update what items are equipped for a particular plate, and claims it's fine when I save it. But whenever I join a duty that wants to use my plate it complains that my gear doesn't match and will instead use the default. Maybe I'm missing something, but I cannot figure out how to force it to update a plate with what I actually currently have equipped and glammed.
Ironically, the original beta implementation of the system was vastly better than its current state. I don't get why they feel the need to force gear restrictions on everything and can't just let us use our adventure portrait for all instant portraits.
Basically, you have to update your gearset to whatever change you make. Then, go to the Portraits menu, and any that has a yellow warning is one you need to update. Just edit it and save it instantly if there aren't any major changes you want them to make.
If they don't match your glamour or options (like visor or headgear staying hidden), it's because you didn't update the gearset. Go do that. Then come back, edit, and save.
Now, why is this menu still a hot mess of user-end software design despite its alleged "Beta" phase having passed and we've had the current system more than one patch cycle?
I've noticed that sometimes it takes a couple of minutes to fully update the portrait, even if it's for something as simple as a color change to one piece of gear. I'm not sure if that's related, but that was a thing before the patch as well. Maybe that is playing a part in this or something. The portrait thing is finicky. Even the slightest change can make your portrait show up as the default. You have to keep it updated by going into edit mode constantly which is honestly pretty annoying.

Nowhere in any of the tooltips does it actually explain that your gearsets have to be 100% up to date with what you currently have equipped, and that any slight modification to them will break the adventure plate. It used to just snapshot whatever you currently had equipped if you told it to update, which IMO was vastly more user friendly of a method. The way it works now just makes it even more of a chore to deal with.
My static save for one person has done everything we can think of and we are still having the default error messages.

Then you have to go back into the adventure plate menu to modify and resave the associated gearset plate to make it update with the correct gear/glamour.

I tried all of that and its still getting set to defaults, the only thing I can think of is that it doesn't like my pose. Only way I know how to help would be to know how the Portraits Menu displays that portrait (does it have a yellow triangle?) looks like and whether the gearset you're using is updated (if it has a gray triangle, it will cause troubles).
I really can't think of anything else...
Twelve forfend, this whole system is a mess.
You need to update your portraits if you make even the slightest change to your character... Hiding helmet, hiding sheathed weapons, Changing any piece of gear even if you glamour it to what was on the plate.
I did at one point when grinding PvP for the Fylgja mount in 6.2 find that sometimes it refuses to show even if everything is correct and you need to make a change to your gear, save the portrait, then change everything back and save it again for it to work. For apparently no reason other than the plate system is about the least stable system in the game.
It is mildly frustrating.
