I've said this before but I think the lack of requirement is because there isn't strictly an enrage, but rather a soft enrage called Sewer-dweller that just makes it do more damage and makes it harder to kill. I could be wrong obviously, but this has been my understanding from the live letters.

The lack of an entry requirement is simply because you can only enter the savage version with a pre-made party. This is similar to how Ultimates don't have an entry requirement.
