    Well, taking the obvious reason for this decision is that this is simply not Earth and thus their way of titles may have different traditions.

    My speculation is that your surname is your family name and thus in many cultures your family name is actually more important than your own name (for instance, the traditional formal way of writing Japanese names is surname/family name is first and your given name second, but due to Western influences the modern way is to write the given name first and surname second), so this might be a possibility.

    This also explains why Sunseeker miqo'te like Y'shtola are addressed by their first name, due to the percularities of their naming traditions, with women's surnames being merely the first name of the nuhn who sired her, and men generally just having a title denoting their breeding 'permissions', or more often, the lack of, and thus neither really have a true 'family' name, other than their tribal letter.

    And lets face it, identifying Y'shtola as "Lady Y" would just be silly...
