I checked my achievements and I have that same problem

And I think it's gotta be a bug considering they said "The achievement "Mapping the Realm: The Diadem" is available once more." back in patch 5.21

You haven't fully explored the Diadem. The achievement is VERY obtainable. I got it a few months ago

For the diadem one you actually have to fly over what used to be all the little starting point islands. post a screenshot of your diadem maps and can probably figure out what you're missing.

Huh...apparently I was missing three of those. Now that's one less achievement I need to do.
