Not a full identify crises, as the NPC's full name is "Jandonaut Fouillel" (cite). That said, most NPC's seem to be identified by their first name only, although some are identified by what appears to be a first and last name (i.e. Apacho Naccho, Jihli Aliapoh, Millith Ironheart or even Moenskaet the Honorbound). There are of course also the NPCs with purely descriptive names (Beleaguered Botanist, Mumpish Miqo'te, or Pissed Pirate).
What is unclear is whether any of the other "1-name" NPCs are identified by their last name. No easy way to figure that one out as far as I can tell.
In any event, I suspect this is a mistake, but I wouldn't put it past Fern to come up with an elaborate reason why this NPC is different or to identify other NPCs who are identified by their last name.

Is everybody ready for an elaborate answer? The mystery of the double name is, as gahoo has already pointed out, a rare case of the NPC's last name serving as his 'display name' (what we call the name that appears above everyone's heads). As you may have noticed in quests, members of the Grand Companies will often use the 'title' + 'last name' combination when referring to each other. To make sure that players searching for that NPC would be able to easily find him/her, the EN team decided to use the last name in the display name, as opposed to the first--the display name basically representing what people in Eorzea call this person.
Some people may also have noticed that the Japanese display names for Grand Company members use the full name AND rank. We considered this as well, but quickly shot down the idea, because (with our current UI) it would have meant malm-long names like:
Flame Private Third Class Crestian Goodwife
clogging up the screen.
For the most part, NPC display names are always first names, because this is what the NPCs 'go by' in Eorzea. There are some who deviate from this rule, (full names, titles, etc.) but these are almost always special cases (such as 'named' heroes including Moenskaet the Honorbound).

Sure is cool that NPCs can have only their first or last names displayed. Wish we lowly adventurers could be able to experience that luxury.

One of the big things that the LOC team has been pushing since 2.0 talks began was implementing a UI system that could support the abbreviation or switching on/off of player display last names. The UI team has been informed, and is currently looking into a way to make things better. We'll have to wait and see what they come up with, but hopefully it'll be a (few hundred) step(s) above what we have now!
Oh man, you know what would be nice? An option to not just shut off our last name, but to shut off our FIRST name and have a title before it, such as Serpent Corporal Shezad.
