Shadowbringer spoiler.
Tesleen the Forgiven. Her transformation scene unnerved me in a way that no other scene in all of FFXIV has before. That's not to say I've never had emotional responses to the game or it's story, but it's really the only time the game unsettled me in that way, both with the scene itself and her design of her transformed form.
I am somewhat terrified of spiders, so anything with multiple legs really.
Particularly when you can hear them scurrying about on the floor....shudders

The giant heads in Yanxia are still the most unsettling for me, I think. Why is it like... it's just a head. Where's the rest of it? There should be more. More of the body, not more heads. That's also part of why it's so gross is that there are just so many of these heads. If it was just one boss it might make sense but no it's like some giant accidentally up-ended their basket o' heads in Yanxia. I hate it.
Part of why spiders startle me IRL is that they make zero noise, so I'm actually a fan of being able to hear them in games. I know what to expect. One of my most vividly horrifying game memories was of some dinky little cave in Silverpine where at the back there was a spider tucked into the ceiling of the cave that I didn't notice until it quietly dropped down while I was going to loot something. Similarly terrifying was the giant-sized monster spider in Terokkar, but I was able to hear that one's legs idly tapping before I came around the hill to see it - at least I was prepared for a spider, just not one that size.

If it makes you feel better, though, their Triple Triad card mentions they're just golems made out of mud.
