I think you missed the part where I made mention of people actively trying to replace the lost members /with friends/ who queued /specifically for in progress Monastery/ only to end up in a fresh instance. This has happened to me several times, once even whittling down the queue time to less than twenty minutes because we simply couldn't fill the empty spots. If this was an issue of people only queueing for CT then the people who were actively trying to get into the instance to help their friends wouldn't have been shuffled into entirely fresh instances either. So there's enough people in the queue to make entirely fresh instances but not enough to fill spots even with people selecting 'join in progress' in their settings? Yeah that doesn't track.
This is the only raid I've had this happen with as well. Ridorana? Doesn't happen. Rabanastre? No issue there. Dun Scaith? I get this more than the tower climb and it's never an issue. Monastery is the only 24 man I've had near constant issues with getting missing spots refilled to the point if it just doesn't surprise me anymore.

Today, in the Vault I noticed how much I fail as a tank (PLD) thanks to my lack of awareness of my surrounding and "multitask" for lack of better words: I died once at the 2nd boss due to the dimensional tears thingys being all over the place (and I am clearly too stupid to place my self correctly to prevent this) and trice during the last boss mix of fire AoE and horse chess piece thing combination and due to the rezz debuff =/. I know that it's objectively not hard to do, but most time I moved out of the AoE, I ended up in on the wrong side of the approaching row of pieces to not make it in time to the obvious gap xD. Be it out of nervousness, lack of situational awareness or whatever. Thanksfully, even after the only wipe, nobody complained and at my third death the boss died a few secs later. I wanted mentally to quit out of feeling bad! I also felt like I ate too much damage overall (with or without oCDs); maybe that's because I never tanked in MMOs before and had a weird-ish false impression in my head. Dunno.
Kudos to the group for entertaining me.

As a SCH, I had a similar situation with a BLM on The Twinning via Expert Roulette. The BLM on this run did the single-target Fire 4 on big pulls. The only noteworthy AOE attack I saw was Foul. When the final boss knocked that BLM out to 0 HP for not avoiding the delayed AOEs, I left that BLM sleeping because I was busy trying to heal the tank after the tankbuster and help DPS the boss down (boss was nearly 0 HP at that time.)Just had a run of Akadaemia Anyder where one of the BLM's AoE rotation was Freeze > Fire 3 > Fire 2 > repeat. No Flare or Thunder IV in sight, although they were using Enochian so there was the occasional Foul at least.
Their single target rotation was: Thunder 3 -> Fire 3 -> 3 x Fire 4 -> Freeze -> repeat. Sometimes they would use Tranpose instead of Freeze to switch to ice stance. No Blizzard 3, Blizzard 4 or Despair in sight, although again at least they were using Enochian so there was the occasional Xenoglossy.
I tried suggesting that they use Flare instead of Fire 2 since they would do far more damage, but they either didn't see it or ignored it. The other BLM was doing average dps (everything looked good with their rotation, but they didn't have any melds) so I just facepalmed IRL and settled in for the slowest run I've ever had. I think Quetzalcoatl jumped 5 times during the final fight it was that slow. It would have been slower, but me and the tank DPS our hearts out to try and make up for it at least a little bit.

ilv checks when please. Just got done with a Qitana Ravel where I was fine with the healer stating they were still new until I looked at their gear. All lv 70 tome gear except for the ilv 55 sch book. They were even synced down and obviously got to that point in the msq. The tank was the only non synced and they were properly geared. So was the other dps who thankfully was a rdm and not a friend of theirs unless they just haven't moved to join them on the same server. Anyway I state hey a better weapon will help you heal better as seeing on the 1st two pulls the tank and everyone else's hp dip so low hurt as a healer main. The tank goes and says they weren't expecting an on level dungeon and that they were still leveling. I respond with the fact they shouldn't have said il55 weapon reguardless. Which the tank went oh come on. I replied with the fact that the game throws gear at ya. No wipes and I did die once but I didn't notice where the stalacties were on the bat boss.
Im not sure what's worse that healer or the one I ran into in a 50/60/70 roulette that had an lv 80 sch book but then the rest was just a hodgepodge of 50/60/70 gear.
What is with WHMs who do not understand holy. I keep seeing in dungeons, a WHM standing away from the group casting holy, where it does not hit anything, and in Trials, casting holy at the boss. I feel like this is what we get from the frustrated healer mains and people who do not know how to play the class.

Earlier today I saw some ungodly thing happen while running alliance.
Queued up for roulette, got plopped into LotA. First time bonus from our group's bard. Everything is fine until we get to the first boss. To note, I was in C.
The tank in B magically gets this great idea to keep the bone dragon in the middle of the room. First time he goes down, the magic starts.
All of the skele mobs bumrush to him, and since he's so conveniently face-down in the middle of the arena, the damage that gets pumped out raid-wide is enough to completely wipe A. B and C nearly die, but because our healers were on point, we barely survive, and then the tank gets the idea to move the boss...by one tile. Needless to say more damage went out, nearly wiping the rest of us and making the healers that were still alive work overtime to keep both B and C alive. Group A never got a res.
The rest of the dungeon went uneventfully besides one of B's dps getting nuked a couple times because danger bongos aren't loud enough.
This one is beyond your help. Only sleep can save her now.
Uh... middle of the room for the Bone Dragon is the norm. Kill it fast, heal through the damage, repeat. Everyone stack on central platform or the first small platform behind the boss to make sure everyone is in range of AoE heals. If your raid's healers weren't able to heal through the platinal phase, that's them being bad or the DPS being too spread out and not the fault of the tank in group B.Earlier today I saw some ungodly thing happen while running alliance.

The old data center arguments between tanking it on the edge or the middle platform died after a couple expansions gave people more than enough hp and abilities to just force their way through that mechanic.
