Some story annoyances, because sometimes I actually hate how our character reacts to things...
When your character just sits there shocked and watches something horrible that they very much and very easily could have stopped happen.
I'm looking at you, tank role quest. Seriously, you're just gonna watch a cardinal virtue slowly approach a dude, stare him down, and murder him within like ten feet of you... and do nothing???? No shout, no *shot in the back,* nothing????
The only excuse I can possibly think of is not being sure if you can survive a fight with him but??? Distract him and run away or something at least, like come on you've fought much worse and much more and your ability to react just vanishes *then?* That merchant did not need to die and I'm irritated that he did.
Also throwing the eyes of Nidhogg off a cliff in HW was dumb no matter how "impassible" that cliff was supposed to be. At least like, think about your plans when it's *appropriate* to. Ahead of time maybe??
Though I honestly can't remember if they did plan that one ahead... just seems like it was kinda poorly thought out.
Also, when your character steps back in shock when the thing said isn't even actually shocking at all. Idk, I get it's intended to be surprising or whatever, but there's gotta be a better emote for "oh wow didn't know that" than :O! SHOCKED GASP! Pls SE.
