Huh, interesting revelation.

But you can't access the forums if you haven't logged in for 14 days on a character. So if your account gets banned, 14 days later you can no longer access the forums, in essence getting forum banned as well.

To answer the OP's question: On Reddit. Also some Facebook groups. Half a dozen or so people who clearly crossed a line, got reported, and got banned. Reminds me of the old days in XI when people swore up and down they didn't cheat or buy gil, but upon further investigation they most certainly did and bragged about it to other people.
It has not been massive waves of people being banned, however, because for the most part the folks in XIV are a comparatively mature bunch and can tell the difference between a joke and harassment, and the preferred method of dealing with random strangers being bad is to coach them if they're amenable or blacklist them if they're not.

If you are banned from the Forums, you can still play the game.
If you are banned from the game, you can no longer access the Forums, as it requires a currently subscribed account to utilize. Your account is no longer subscribed if SE bans it. You’ll probably have the 14-day grace period that people who unsubscribe have, but if the ban is permanent, you won’t be able to access the Forums after those 14 days are up.

The changes to the ToS definitely didn't result in super aggressive banning or reporting, as far as I can tell. I've submitted a few reports myself (one of which I was informed they received multiple reports on, and so far I haven't seen that person log in for almost a month now), but certainly haven't been disciplined for any bad behavior. Usually I'm quite Good, but there was one day like a week ago in LotA when I lost my temper with someone who was probably trolling and I definitely used a decent amount of profanity, but haven't received any disciplinary action for it. So it's sort of a two-step thing; you can't get banned for small things if people don't report you, and then if they do it's up to whoever gets the report to decide if it's worth banning for.
Maybe the ToS cover a wider range of infractions now, but people don't seem to be reporting any more hastily than they were before.
And also people got reported frequently on the forums from people that didn't like their opinion and they've got banned for reasons that wouldn't even give a warning in other forums because there was no harassment, insult or what ever. SE have their own ban politics and they are not even clear about it, yes they have their ToS but how they get executed differs extremly and that should not be.To answer the OP's question: On Reddit. I know someone who was banned from the forums for this reason. Could not appeal either. They showed me the posts that (likely) did them in and honestly it was no more volatile than how outwardly rude people can get. But all it takes is someone to actually report. And also people got reported frequently on the forums from people that didn't like their opinion and they've got banned for reasons that wouldn't even give a warning in other forums because there was no harassment, insult or what ever. Proof cannot be given if GMs cannot state a reason or line of text that gave you a penalty in the first place. Can't ask for evidence when the evidence is never given.
IE, it could be a long term investigation of something that happened before the rules change, and now it's in violation due to the new rules.

Also it's against the rules to talk about bans. Even if people did get banned, they wouldn't risk it divulging it here.
