I think in a lot of cases for me, the Medica II is less for the tank and more for the dps who always run out into Narnia and never dodge anything.

Regen is pretty potent on its own, especially if boosted with Largesse. Like you, I tend to use Medica 2 when I know other players are going to be taking damage. Whether this is due to unavoidable AoE, or incompetency, either of these will flag the use of Medica 2, and I don't cast it to help sustain a tank when I have so many abilities that already take care of this.
Regen can also be cast on the move. There is a slight moment of animation lock, but it is so fast that a telegraph will have to catch you at the right moment. Medica 2 is the opposite, and not only locks you in a long cast time, but also takes away your mobility. I can't even begin to tell you how often I swiftcast this ability, because of how vulnerable it leaves you.
Regen and Asylum to help sustain a tank however, are a no-brainer. If DPS are taking damage while Asylum is up, it like, "Yo? See that flashy big bubble thingy? Stand in it and it will heal you!"
