I like to establish a strong lead so that there's less chance of having to dip back into tank stance later on in a fight.
Eye is ignored there because the an entirely new SS/BB should generate more emnity than an eye-buffed SS/BB, if my understanding is correct.

Just kidding. Seriously tho, 3x BB is too much. Enmity is party duty and not tanks alone. Ask your OT to shirk you, or DPS jobs to use diversion, then enmity is no concern.
Yet your opener should look like this or this if you need more enmity.

I use Equilibrium as a poor-man's flash when tanking dungeons, but it never occurred to me to use it as supplementary aggro during the opener for some strange reason.
I'll give em a shot next time I'm running stuff. But typically the only time I'm MTing is in pubbed content anywho (Ex primals, omega normal, etc.), and you can't trust randoms to manage their emnity. =w=)a

6 Fell Cleaves is cute and all, but you're actually hurting yourself in the long run. Just use IR + Berserk on your second combo and go with 4 + Upheaval and an Onslaught.
Here's what I use for Exdeath when I don't have a Ninja in the party, and need to hold enmity. Starting in Defiance:
Tomahawk + Equilibrium + Deliverance
Heavy Swing (Infuriate) -> Maim (Thrill) -> Storm's Eye
Heavy Swing -> Skull Sunder (Berserk) -> Butcher's Block (Inner Release)
Fell Cleave (Onslaught) -> Fell Cleave (Upheaval)
Heavy Swing -> Maim -> Storm's Path (Holmgang)
Fell Cleave (Rampart) -> Fell Cleave
Heavy Swing -> Maim (Convalescence) -> Storm's Eye (Reprisal)
Heavy Swing (Onslaught) -> Skull Sunder (Infuriate) -> Butcher's Block
Heavy Swing -> Maim -> Storm's Path (Upheaval)
Heavy Swing*
Heavy Swing -> Maim (Onslaught) -> Storm's Eye
* You will end up with a free GCD at the end, but it's better to end with Storm's Eye so that the buff is still up after Flare. This could be a Fell Cleave, but Berserk will be up after Flare too and it'll be better to use it there.
Now this here is overkill when it comes to enmity, but it guarantees I still have a big lead regardless of party composition, and also makes sure I end up with 80 points on my beast gauge.
With a Ninja, the combo changes towards the end. Instead of using another Butcher's Block, I just use Storm's Path and swap the two Onslaughts for an extra Fell Cleave, again leaving me with 80 points.
So you see, there's no need to stay in Defiance, OR to use a repeated number of Butcher's Block combos, just to get an enmity lead.
