I'm not all that experienced with modern MMOs either, but this game has far quicker "gear inflation" cycle than any I've played. I suspect WoW is similar now, but I haven't touched it in ages. Coupled with the typical weekly Tomestone lockout and the result is, yes, if you're not constantly pushing the content, you'll end up "behind" the gear curve. It won't matter in general, but you will fall behind a bit.

As for me personally, the dull content cycle is precisely why I'm subscribed fewer than six months a year, on average. I can't speak for others, but I know it's been a frequent complaint on the forums. Every so often SE will introduce something new and different, like Palace of the Dead, but for each successful bit of new content (PotD being one), there are more that fall spectacularly flat upon initial implementation (Hunts, Diadem). So if you aren't grabbed by the gear grind (Tomestones or Relic), or addicted to Crafting / Gathering (which are a lot more grind-y), you'll probably end up bored for much of the update cycles.
