The more I think about it, the more I suspect this is a programming error and not a hardware error. Something about the MSQ instance in particular must have a memory leak or poor garbage collection, causing it to consume far more resources than it ought to.

It could be something like that but having designed an written high transaction servers I would expect something like that to be fixed fairly quickly and to also result in them shutting down the MSQ instances altogether until it was. SE says they have plenty of instances but are restricting them which is why I lean more to a major capacity miscalculation and under-sizing the server requirements.
