That's the thing. It could be anywhere. In a 4 man dungeon, or while sitting in the Mist at my FC house with no other people there. It's completely random. It gets worse when using skills. For example sitting at my FC house right now and using Benefic II, I get the frame rate drop. This never has happened before up until Patch 3.4
