If I'm reading you right, you have two Square Enix accounts and two PSN accounts? The PC version is on one SE account and you have the PS4 version on another Square Enix account, but that's also on the wrong PSN account as well?
I'm not even sure where you should begin trying to fix that, but you probably want this link for at least part of it (assuming you are in the EU based on your server):

That's for reporting moving the game from the new Square Enix account to the old one (I can't see them moving the old one to the new one since that came first). But you'd still need to reset your old account to get access to it. And contact them to unlink it, but then also probably have to involve Sony as well since the licence for the game is with their system (and I'm not sure they'll let you move it).
Probably best thing you can do is ring them and try and explain all this and see what they advise :S
Sorry this isn't a lot of help, but this sounds really complicated and you're really going to need to talk to SE directly.
Could this part of the problem be fixed by logging into your old PSN account when you want to play PS4 FFXIV? You can set up multiple profiles on a PS4 that each connect to a different PSN account. You'd just need to switch profiles, one for when you want to play this or any other games from our old PSN account and another for any that are linked to your newer PSN account.
That still leaves the issue of the two different SE accounts, the one made when you set it up on PS4 and the newer one made when you set it up on PC/Steam. Hopefully SE's customer service can merge those for you.
