I don't get the issue. Male/Female can be changed easily enough in game so any transpeople wouldn't need to cross dress... they just change gender?? Maybe I'm missing something.

Well I can't say anything for trans people but
That doesn't mean with a male character I don't want to dress up in a dress or skirt or whatever.. or even seemingly genderless items like the Bunny Crown yes I'm still mad about that so

whatever.

Also just like some people find super manly muscley men attractive, maybe others find extremely androgynous people attractive so....no it's not as simple as "changing gender"

The game really can't be all things to all people. Hell summoners have a much better argument regarding their EGI.
