I'd like to throw in my issues here too. I have spent too much time trying to fix this issue. My current specs:
    i7 5820K
    EVGA SC acx 2.0
    1000W PSU
    16 GB DDR4

I have tried the following and had the same behavior:
    -Windows 7, 8.1 and 10 fresh installs
    -Graphics drivers: 353.30 (current), 350.12

I had tried running with driver 350.12 because I had never had this problem before heavensward running that version.

Just wan't to drop a note here. The issue lies mostly with EVGA Cards. They allready posted on the official EVGA forums thaty the will set up an davanced RMA for their 980 FTW models. But i think the problem is very widespread under EVGA cards, especially under overclocked ones. I have an 980 gtx FTW Ac 2.0 and had crashes all over the place. Contacted my retailer this morning getting the answer that i was the 5 customer with the same issue this week with EVGA cards, and they switched them all for an equal model from MSI. I will get mine switched tomorrow and will update here if it fixed my issue. At the moment i run my old 680 gtx, with no crashes so far under dx11 high settings.
Sorry 4 typos, don't have much time right now.

I`m really worried that i'll soon be in the same boat as you, i've ordered a EVGA 980ti SC acx 2.0 , 1 week before heavensward release, and i'm waititng it arrive to change my 970 , but since its a SC one and from EVGA im really really worried ill be having the same problems im having with the 970. I cant cancel the order or change products yet because it was already shipped.

If i still get the same errors on dx11 with it, ill try to change to similar model from another manufacturer. I really liked evga cards, but after this experience i guess im not buying from them anymore

I've been having the exact same issue. GeForce GTX 980..I upgraded from my HD Radeon 7970 and the game will run for hours sometimes, sometimes minutes. It just cut off on me twice in a row within a 10 minute timeframe.

Yeah, I've just took it to my local tech store to check it out. I'm basically BSOD on ALL my games now, and it isn't pretty. Upgraded from an AMD 7850 last week, and got a EVGA GTX 970 SC card installed. I've had nothing but trouble with it. It seems as though the 970/980 EVGA SC cards are having serious issues, according to the forums on here and on reddit. I'm hoping I can swap my new graphics card out for the MSI version, which is vastly more reliable I hear. Who knows. It might not even be my card, it might be something else - something more sinister. I'll keep people updated.

Thanks. I'm actively pursuing a solution myself. TBH FFXIV is the only game that I'm having a problem with, but I've been readying that others have been having problems with other games.

Okay, decided to go back to the store and get a replacement card. I've decided to go with the GTX 970 MSI brand, just in case. I've kept reading there's a specific problem with the EVGA SC BSODing, so I'm praying that it was a faulty/technical issue. I'll keep everyone updated.

For those still crashing on 11, try to set tessellation to normal , and play on full screen, everything else i had maxed out. I played yesterday for 4 hours without crashes, and didn't crash when exiting the game too.

But for those crashing on 9 , i don't have a tip to give

Yeah let us know how that works out. So far the only solution I've found online is to get a replacement card. Everyone else is still troubleshooting the issue :/

Okay, so I'm back. This morning I took my PC back to the tech store and told them the problem. They checked everything out, and said it was 'probably' a faulty graphics card. However, I asked if it would be okay to change brands because I'd heard the MSI cards were more reliable with the issue I was having, and so I changed it to an MSI GEFORCE GTX 970 OC edition. I got my PC back this morning at 10 'o' clock, and I've been playing for the past 8 hours with zero issues. It leads me to believe the problem we're all having, mainly, with the BSOD and black screen crashes, are directly related to the EVGA 970 SC cards in particular. I only got the EVGA SC last week from an upgrade of having an AMD 7850 and all it did was give me trouble. Having switched this morning to MSI, I've been left with a problem-free experience. It's early days, but it's looking positive. I really hope there's a fix eventually for anyone else that's got those cards in particular - and even if you haven't, a fix somehow. It's beyond frustrating.
