Do you have to collect 5 swimsuits on each character that you want to have wallpapers of? Or are you limited to just the first one you unlock the with? How does that work?

You need to get 5 to create wallpapers. When you create the wallpaper you can choose from 2 pose, 5 swimsuit, 4 background and 4 resolution.
