It's gonna be bought for sure. Likely will either have to buy the physical copy or pay for a code to unlock the content. If SE is smart they will simply charge for the unlock code and offer the expansion data as a free download.
It will most likely be a paid expansion ~$30.00-40.00 like most MMO expansion packs. As far as content, there have been several times when the dev team said that 3.0 will be about as big as 2.0 was at launch content wise. So it will be pretty big. Not only in content, but we will also get new classes I'm sure we will see at least a new tank and healer type. Maybe even the much rumored "hybrid" classes.
We will also see a level cap increase, so expect to see at least 1/2 the new dungeon content aimed at level 50+, as well as a new endgame progression.
Last couple expansions for FFXI were digital only so I expect them to do the same for FFXIV.
You mean the add ons? A Crystaline Prophecy, A Moogle Kupo d'etat, and A Shantotto Ascensian? Those were just little addons, not expansions. I have a disc for every expansion except Seekers, since stopped playing right before it came out. So unless you mean the mini-add ons, yes those were digital only.

I played a few games, for example Aion (when it was sub based) 2.0 expansion hit and it was just a big update (yet still an expansion). They sold a physical copy aswell if you wanted to have the physical disk to install it directly.
The way I see it release - buy, uses that money to fund development of the expansion which you again buy... etc... patches in between get funded off of the sub fees.
