Harassment.
Sure, it's probably not verbally (though if you offer them advice, usually it will become), but when some people are playing that badly I can only assume they're trolling.
There's not knowing mechanics, or making a mistake, then there is playing the game with a Power Glove while a cat bats your keyboard and mouse around. I'd also consider it harassment for someone to keep screwing up and not say a thing; I've had plenty of parties where we wipe because of one person, and I really want to kick them, and then they apologize. I even give those people commendations, because seeing someone actually take fault in this game is pretty damn rare. Continually messing up and saying nothing is just one tiny step up from messing up and pointing the finger at someone else, in my book.
Lesson is, always offer a bad player advice, because they'll either stop sucking, or verbally assault you giving you a perfectly reasonable justification to kick them.

Don't take the list of "reasons" so literally. Sometimes people are so bad they're beyond any help you could give them while in an active duty so the best solution for everyone is to just remove them. Doesn't matter if SE didn't make a specific category for "brain dead". There are so many different reasons you could (or should) remove someone, there's no way SE could list every single one so they gave us the four most common reasons used for removal.

No, only if they consistently ignore the guy who tanks in Second Coil when he's explaining to them what they should be doing different and why. I can understand having trouble holding aggro, but if a tank is refusing outright to use Flash or even Savage Blade when the rest of the party is telling him to, that's an absolutely justified use of vote-kick.

A tanks very first dungeon ever, in all of Final Fantasy XIV, and he gets kicked by some twat of a lvl 50 player just because he's having trouble learning how to play his class?

Do ME a favor and read my posts before you decide to bawl me out. I'm talking about people who outright refuse to listen to a single word of advice when someone with a literal year of experience is trying to explain to them how and WHY things need to be done. "You need to use Flash or the other mobs will beeline for the healer." "Riot Blade is actually crappy at picking up enmity, you should use Fast and then Savage Blade instead." "Try marking the mobs so the DPS know which one you're focusing hate on." When I spend fifteen minutes explaining these things over and over and the tank insists on doing nothing but spamming Fast+Riot, I have every right to boot him out.
Nah, I fully expect to rip hate off tanks even when they do flash and mark targets because low-level threat generation can be awful (especially against pro dps). Good tanks stand out and get my commendation though.
