I love asking if anyone doesn't know the fight/dungeon and getting no response 'till things go wrong; most players seem to need to learn how to communicate more, not just one set.

This.
Hate Cutter's Cry for this reason, ask if they wanna kite or grab the princess mobs *crickets* everyone dies from going one way or the other, get all the blame after.
I've learned you often have to be more specific than that. Cutter's Cry was used as a good example. You ask me if I know the fight with the princess, I'll say yes, assuming that the healer is going to kite the adds, whereas the healer might have always had the tank pull the adds. If the notification comes up that someone in the instance is new, I'll quickly explain all the fights as we get to them without asking "is anyone new?" and putting some new kid on the spot.
