A big problem with expecting people to have watched/read guides is that for some people that is of no help whatsoever. Some people can only learn by doing the thing. Some people are slow learners. Just because YOU learnt it quickly doesn't mean it's going to be the same for everyone else, I'm afraid. I've watched the Titan HM videos like ten times and still can't remember half of the fight xD
I'm not denying there are some truly awful players out there - me included! I'm not usually an end-game kind of girl =P Which is why I don't DF for hard fights, I'm slowly working my way through them by PFing learning parties.
The problem of people who haven't watched the fight also arises when some people who are fresh 50s expect to be able to just do early end-game content like HM Primals and CT without looking beforehand, as that is how they've done the entire rest of the game. It's even more of an issue now because, to an extent, people can be carried through HM mode Primals due to all the i90+ people farming them for glamour weps so they stay firm in their belief that they can just PUG everything. Then EX Primals happen xD

I watched Titan HM by mtqcapture (who does really damn good videos instruction-wise) like three times before going into the fight. Yeah, I 'knew' it. However, putting it all into practice? Another thing entirely.
I don't believe this. Everyone can benefit from watching a video/guides. If not then they most likely can't do the content either way.
Positioning is huge in rotation/phase fights and it should be easy to pick up from watching a vid. Fights like Titan is less about positioning and more about just dodging but you can probably learn simple stuff such as stacking for plumes.

I was trying to find a decent instructional video for Leviathan Hard (not Extreme, just Hard) last night since I was finally getting around to it. Every 'guide' I could find was just like "yeah uh watch for splashes and run from them, one tank here one tank there, grab adds duh, I guess there's a thing you do to block tidal wave but idk lol someone else always does it." - and that was the most informative I found. Even after doing it myself, I still have no idea how the hell the Tidal Wave mechanic works or how/when to activate it or what.
Finding actually good guides for some stuff is harder than it sounds.
Well considering Levi Hard is so easy that you can walk in blind and come out on top, most people don't spend a lot of time learning the specifics of that fight. That's true for most encounters in the game. There are no guides for levi hm because there isn't much to the fight really. I went in DF and asked what the fight was about and i was told its nothing special and to just heal awayI thought they were being mean, but it was pretty much the truth. Like you I found no guides for it.
For videos you just have to find someone who makes sense to you and stick to their guides. I don't like a certain famous person's videos at all, they seem too chaotic to my brain. However, I managed to one shot other content with videos from a less known guy, his guides were perfect.

It's just like battle on the highbridge.. it's kinda very very straightfoward tank and spank to the point that wipe is almost impossible. Played Levi HM first time as a BRD, no particularly what on earth is going on and the team one-shot it, same thing for highbridge. Heck, went in as healer and healed a couple of first timers and still, no wipes. Maybe that's why guides for these instances aren't very detailed.. cos there isn't much detail.

New players (especially those who are new to MMO's) can find it difficult when hitting on endgame, but once again (not trying to sound like an elitist, cause I'm not) I've been through the same thing once, and so has every other player further in the endgame... I did the mistake of entering Ifrit HM without looking up the fight, and I payed for it... After that I looked up every other fight before doing them, except for new fights from patches. New players should learn this after doing Ifrit HM without watching/reading up on it. Even if you are carried by ilvl90 people, you should realize you didn't do a good job, and that you should be more prepared the next time...
Okay, while Levi HM is not exactly the best example to have used there, here's the important part:
That's the hope, but there's a difference between just "oh he's attacking this" and "he's attacking this and the DPS aren't attacking it because the boss' aggro is controlled through mob kills".
Through a lot of endgame content, you won't really get by on just "oh yeah I think at this part you do something like this?" as opposed to knowing that you move to this location because it's a safespot or because this way all the targetted AoEs are stacked and you can dodge on that tiny ass little platform etc.
It's easy to find a video; it's tough to find a video that actually gives a player enough understanding to replace genuine experience or being able to ask 'why'.
