Am I ready for Titan HM?

Only thing I'd say is worry about your weapon first. Accessories would be a good upgrade after that but you should be fine with just a weapon upgrade, even if you're only going to use it for one fight. Ifrit, Garuda or the i70 crafted weapon (HQ) should be fine.

It will be very hard to hold hate against the healers otherwise.

Congrats for the win, OP! For other people interested, I would have advised to try and see, finding a good group is more important, but if you see you have health problems (really good healers can solve that), you can do other things while you gear up, do CT for the loot and tomes, obtain more DL gear, buy Wolf Amber accesories (ilevel 55, good basic attributes) if not very expensive, for the accesories you don't yet have the DL. And go back to Titan HM with higher gear that would make your healer's lives easier and give better chances for the group.

Tanking Titan is super easy. You don't even need to dodge bombs, just pop Sentinel or Rampart. Holding hate is almost never a problem. In all my runs I only lost hate when 1) I died 2) Bard let loose before I could get off Savage Blade. Just tell your group "lol let me establish hate first xD" and you're good to go for the entire fight.

If you find yourself getting bored, try numbering the first bomb that lands so people have an idea where the chain is starting. If you see someone's pet is about to die give them a heads up. Otherwise, you'll find yourself not really doing anything but mashing the same buttons over and over.

More dl would be better but really Titan HM is more of a dps and healer fight then anything.
The fight outcome all depends on how well dps can take him down

Would you guys say I'm ready for Titan hard mode with the gear I have at the moment? (I know the tactics, etc.)
I hear people say to obtain "Ifrit's Blade", but It seems the sword I have at the moment is about as good. (weird)
If this won't do, what gear pieces specifically would you guys have me obtain? Thank you

Weapon should be fine. Healers should be mitigating damage with Aldo or stone skin, to reduce threat anyways.
