off the top of my head the only boss in a RPG i can remember having difficulty beating is Dark Bahamut in FF IV. I'm pretty sure i almost broke my GBA in a fit of rage.
EverQuest: Kerafyrm, The Sleeper. It took over 180 people at the time of the first kill, many people lost several levels, took over 3 hours.

And they won with an exploit to boot. Pretty sure that event wasn't supposed to be completed though. And from what I heard it ticked a lot of people off because it made the event before it go away.
I'd say you're probably going to see a lot of "hardest bosses" from the 8 and 16bit eras of gaming. Whether it's due to game glitches making it difficult or just plain old difficulty you won't see stuff that hard again unless it's a majorly niche title.
The hardest fight that I can recall for me though was in Valkyrie Profile for the PSX. I don't recall the name of the boss but it involved going back in time to a city's past to find out why it went apocalypse. The boss fight you have to go up against there is very tough if you didn't expect it as he dropped a very powerful AE on your party very frequently.
Probably some sidescroll space shooter on max difficulty with projectiles from every corner.
Shredder from teenage mutant ninja turtles arcade nightmares of him splitting into two and his life points are crazy, remember popping in quarters just to beat him and it took forever.
The other is Gill from Street Fighter 3rd Strike.
