It'd probably go a little something like this:

Nude patch.
Super Nude patch.
Naked NPCs.
No underwear mod.
Sexy Miqo'tes mod.
Gary's Erotic Lalafell mod.
Realistic Water.

It isn't on par with Skyrim till you have a High res, ultra realistic Cloaca mod for chocobos, to fit your realism needs. The horse version was one of the few times that the net actually left me dumbfounded.
P.S. For the love of god, if your at work do not look up the horse mod. If you aren't go for it, but don't say I didn't warn you.

I could go for a Macho Man mod. Change those toads into Macho Man and rename their body slam AoE to Atomic Elbow... brotha! Sky's the limit!
