Kicking when they repeatedly die to AoE in Titan extreme. We kept booting such people out of our Party Finder pug group until we eventually had 8 people capable of dodging and got our titan kill.

Almost kicked a person from a CT run, 4 fights in a row they were mysteriously locked out of the battle, standing at the line. They were part of the conversation pre-fight every time, but they kept getting locked out.
Same, though I did think about kicking the tank last night >.> I was queuing up with a blm in my fc, doing cutter's. A little bit in, I notice that I'm tanking more than the war, so I look around the mob to see what he's doing.
Spamming maim >.> That's it. So I mention it to him that he can keep hate better if he uses his other skills, got no reply. Wiped a few times on the first boss, did the 2nd boss fine, except the fact that I, the drg, was tanking more than him. Thought about kicking because maybe he was just trolling, seeing as how he had 2 50s already. Continued on, got to Chimera. He ate every Ram's and I had to tank most of the time. So then I ask him again and put his rotation in translator and get, "I was until you told me to spam maim" then argues with me that I told him to do it, yet he was maim spamming well before I pointed it out lol We eventually won, though he kept spinning Chimera :/ Got 2 commendations thoughlol

My first kick was last night in Tower. We were on a second go on Behemoth King when I notice that I was the only one healing in my party. Looked at the party list, the other healer, a white mage, had taken every tick of floor damage since the fight began. Didn't bother healing them. They died, but given how zealous I am at healing she was our only non-Meteor death. Left her for dead when we got the win but I wasn't about to solo heal Quake so, out the door she went. Felt good.
I kicked one dude not letting others watch their cutscenes in Praemotorium, after that the run went smooth and more friendly.
I think that's the only time I've used it.
Have done it only once so far, to a conjurer in Toto-Rak. From the start, he kept harassing our tank (a warrior) to not use Overpower and still keep hate, stating that if he could sleep mobs he would've had more time to DPS with Stone and be less busy healing him, because "1 tank 3 dps is better". Needless to say, the tank hard a very rough time keeping hate without overpower, against my archer friend and my carbuncle. Not to mention, the conjurer's idea was pointless, as what few extra damage he would make with Stone, was counterbalanced by me not being able to cast ruin, as I had to heal tank, carbuncle and archer when she'd get hate. He seemed to pay little to no mind to HP pools. The conjurer complained, and I said that he could leave if we were so bad for him, as we could easily trio the dungeon to the win just with my arcanist healing. He says he doesn't want to leave but just "that people do their job right". Nice to say after you order someone to not use their own class skills.
The kick came later than it should've in my opinion. After the second boss (which we won at the 2nd try, because he hadn't healed enough on the 1st), he claims the loot and just says "afk to pick up kids. kick me if you want". That's when we did it, and everyone was glad. Wish we had done it sooner. He didn't deserve that armor for what he did and how he did it.
Oh and, we still won with me as main healer afterwards. Yay!
Haven't kicked anyone yet but really need to start doing so. I'm getting quite fed up with general asshattery in groups. I don't mind players that may be learning, make mistakes, etc. But someone being a jerk and ruining the whole run will get my vote.
One question....does the party leader have to initiate or can anyone? I really wanted to kick someone on a CT run but didn't have time to fumble around figuring it out.
I only put up a vote if people are clearly violating one of the given reasons. Needless to say I really haven't had to vote kick anyone other than the occasional disconnect that lasts quite a while.
I have had people try to bait vote kicks by either asking for one and, when rejected, they just /sit and wait. I will note I have only had tanks pull this stunt. In those cases I usually get agreement from the other remaining members to note vote kick and have fun in the dungeon for at least 40 minutes up to an hour. My thinking being if the tank is going to waste the time of other members, particularly DPS that have high wait times, their time should be wasted too. Fortunately there is a lot of fun that can be had in many instances without a tank.
SE can be kind of weird in how they enforce rules so I do not want to initiate a kick that could end up being seen as abusive. If I have a significant issue in how someone is running content, I will just leave if I feel it is a lost cause.
Using the kick feature because someone isn't good at the game is considered harassment and is reportable. Unless if you're talking about a pre-made and leaving the instance, kicking, then replacing.
Queued up for CM for FC member's story mission with 5 others. Got a Paladin who bitched about him watching cutscenes and said "roulette is for tomes asshole don't use it for story" which I then explained to him he had it backwards. Either way he initiated a boss fight without me or the smn there and we were locked out. After that fight he asked why was I not there in the fight and I explained there was this magic barrier preventing my entry.
He said "I can do magic too" and proceeded to equip ghost costume and do nothing the whole dungeon. Just before last boss I said "Hey troll wanna see some real magic?" We kicked him but he returned 3 times and was voted out each time until leader stopped looking for members. That was the first time I've used vote kick.
