My FC mate had this happen to him the first week of CT. Passed on WHM Belt and he still ended up getting it. It might not be a common bug but it is there, not to mention CT's loot system has other issues that cause headaches for people, such as people ending up with unwanted loot from Acheron because the party left before they could even click pass due to loading (more likely to happen to ps3 players).
