As requested, this is how I have my crossbars setup. Be warned, it might look crazy and inefficient to some people, and honestly it probably is. It's just how I set it up and got used to it, I have several abilities that require me to alternate between right and left trigger, this gives a weave-like feeling during combat which I feel helps with pacing. This may not work for everyone, but it has for me. I'm currently on turn 5 in coils.
Set1:
Set2:
While running to the next group of mobs, I will hit Left bumper to switch to set 2, mark up with left trigger then switch back to set 1.
Left trigger+B for shield lob, Right trigger+Right on D-Pad for Circle of scorn, then Right trigger+Y for flash.
Without a trigger held down, you can cycle between targets with left/right on your D Ppad, so my basic Rage of halone combo, is:
Right Trigger+X for Fast Blade
Right Trigger+A for Savage
Left Trigger+X for Rage of Halone
Then I'll weave in
Left trigger+Left on dpad for spirits within (unless im saving it for something specific)
Then Right Trigger+right on the dpad for circle of scorn whenever it's up, unless it's near the end of a pull.
Right trigger+B for riot blade
Right trigger+down on dpad for shield bash
Provoke is right trigger+up on the dpad. So if I wanted to provoke+shield lob it would be right trigger+up, left trigger+B. Or if I was trying to use a combo'd rage of halone it'd be right trigger+up then left trigger+X
Then I have my defensive cooldowns, mostly on the left trigger.
Left trigger+Down on dpad rampart
Left trigger+right on dpad for sentinel
Left trigger+up on dpad for foresight
Left trigger+Y for convalescence
Then I have bloodoath, awareness, cover and bulwark in set 2 on my right trigger.
I have hallowed ground on my hotbar to avoid it accidentally being used.
I'll be happy to record a dungeon run, which would people like to see?
Oh, and for raiding, I have push to talk bound to right-bumper, I don't have to switch to set 2 very often during fights so it's not an issue that it sometimes switches to set 2, only takes a quick press to go back to set1 again.
