First instanced dungeon I remember came out with Everquest Lost Dungeons of Norrath (LDoN) if I recall correctly. (even hated them back then as well lol)Istanced dungeons ? Always existed. FFXI had a lot, even dynamis was an istance.
Quest based progression ? you don't wanna know how it's old. This is probably the only thing FFXI invented, the mass open world grind to exp (and it had a lot of flaws, let's not forget)

I agree with most of your points, besides the 1.2x battle system, gearswitch, and no instanced dungeons. I loved XI but I couldn't handle another 'auto-attack' based battle system which is why I only lasted a week in 1.0. Gearswitch to me was always a silly concept. And removing dungeons from endgame is a step in the wrong direction. Removing options from endgame is a poor solution, we should instead have more options with his to spend our time.
Oh the irony.
Considering SWTOR has made more money than FF XI (exponentially) for the past 5+ years of XIs life in the short timespan that SWTOR has been around, if making money really is the goal of games, and making money over its life, even a wowclone that goes F2P is a better business model than a niche-focused game that burns "bright" for a short year or two and then peters out.

This is not a WoW clone, just like every game it's not a clone because they have a hotbar and tiered progression. also, if you say this is a clone, isn't a contraddiction asking to clone instead a different game ? Because you feel FFXI it's better, a cloning is a cloning, period. And you have no guarantee nor the right to say it would sell better.
edit: say "i dont say it has to be XI-HD" don't change the fact it's what you asked.Also, "but it should more unique" following that sentence it's another contraddiction.
