you want the perfect MMO make it dynamic:
Dynamic MMO.
the dynamic MMO unlike the current MMO's is constantly growing with the players.
starter cities at launch are little more than villages with some form of central government.
the parts of the city are actually built by player interactions as tradesmen jobs.
the projects are voted for by the current players. so would be the type of.
to explain this a bit more in depth we can use the example of a guild house being proposed.
in the city hall. the players would have chosen a guild hall from a list of possible projects and voted for it with a clear majority chosing this project over others to be currently worked on by the city. once a project has been chosen by vote to be built it goes to the active projects list and the players using differing trades jobs contribute to its accomplishment firstly by furnishing the materials, ore, stone and lumber which each project has a list of needed materials such as: 500 planks, 1000 stone , 200 ingots of iron ...etc... the interior decorations and furniture can also be made by trades. this kind of building by majority choice gives players the additional feelings of accomplishment as the starter city they are in advances .
mob placement is also dynamic the regional mobs have a static spawn but at first each area needs to be pacified of much nastier monsters who have not suffered predatory encroachment which the players are the predators. some would be strong enough to need raid like numbers or as in ARR the big nm fates with large numbers of players to kill off the boss and make the region generally safe for farming the easier mobs.
as the players open areas dungeons get discovered these also progress as the players delve into them either clearing them completely and creating safe mines or connecting to much nastier content....etc...
the limit to a dynamic game is only in how much effort the maker is willing to put into it.

Sometimes I wonder if the best MMO isn't actually going to be an MMO... things people complain about when they're playing an MMO tend to be particular to MMOs, while things they like tend to also apply to other games.
Judging by so many of the threads on this and most likely every mmo forum, the only good mmo is the one they left behind to play the current mmo which they're complaining about.
Perfect is a false concept.
Or one that closed in the case of SWG or CoH, or in the case of FFXI got changed to something radically different than what it was.
Haha a world with just a bunch of random colored shapes running and jumping around would be hilarious to see
Meh, the perfect MMO passed with the passing of MUDs.
